PFG would like to make this process as easy as possible for you by providing all the forms and information you will need to initiate an LTD claim.To ensure this process goes smoothly, it is important you respond to all questions fully and accurately, and send the forms back to Standard Insurance Company as soon as possible -- you should not wait to file a claim until the elimination period has passed. The elimination period is the period of time between the onset of a disability and the time you are eligible for benefits.HOW TO COMPLETE THE STUDENT DISABILITY CLAIM FORMPlease follow the instructions outlined below:Student's Statement – This section should be completed by the student (the claimant).Authorization to Obtain Information – This section should be completed by the student (he claimant).Attending Physician’s Statement – You (the claimant) should complete Part A.
